Patient Monitoring at Home
At Virtual Logix, we have access to state-of-the-art technology and interactive software to keep you, the client, in the center of the circle of care. We schedule a dedicated monthly encounter to review your vital data, medications, comorbid conditions (COPD, CHF, DM, etc.), and concerns for Specialty involvement. We also provide up-to-date reports for follow-up with your PCP. We send incident reports monthly to your PCP so they can be informed to make the right decisions.
